Traceability Systems Prevent Disaster - Is Your Company Doing Enough?

Recently, Hospira, Inc. announced a voluntary user level recall of a single lot of medication, Hydromorphone Injection, USP. The reason for the recall is that complaints were received regarding the level of pain medication included in a single Carpuject dosage, which contained more than the 1 mL fill volume indicated on the label. Hydromorphone is an opioid pain medication that can have fatal consequences if a patient overdoses on the drug. No reports have been received by Hospira of any adverse effects on patients to date. The company has begun an investigation according to the FDA's website to uncover what caused the lot to have the incorrect dosage. The Hydromorphone Injection, USP, 1mg/ml (C-II), 1 mL fill in 2.5 mL Carpuject™ , NDC 0409-1283-31 lot was distributed to a limited number of hospitals and wholesalers in 29 states during the months of September and October of 2011, according to the press release issued on May 12th of this year. Patients who overdose on opioid pain medications may experience respiratory depression (slowed or suspended breathing), a slowed heart rate and/or low blood pressure. Some of these symptoms can be life-threatening. Hospira warns anyone with inventory of the recalled lot to cease use and distribution immediately, and to contact Stericycle at 1-888-912-7093 to make arrangements for return of the product.
